Gingivoplasty and bone grafting

Gingivoplasty and bone grafting are surgery techniques, which allow us to solve medical and aesthetic problems of our patients. These techniques are used in periodontics, implantology, preparation for dental prosthetics and aesthetic dentistry. The surgical procedures in Meerhof Dental Center are implemented with the maximum care for the patient.

The purpose of the gingivoplasty and bone grafting in dentistry

The necessity of bone augmentation or gingivoplasty, as well as that of soft tissue extraction is evaluated by a relevant treating doctor: periodentist, implantologist, prosthetist or surgeon.

Surgical indications:

  • Gum deformity resulting from periodontitis or periodontosis
  • Gingival hyper- or hypoplasia or irregular emergence profile (cosmetic issue)
  • Deficient soft tissue contour preventing implant placement
  • Ridge deficiency on sub- or supra-maxilla preventing implant placement
  • Necessity to create an aesthetics-driven natural gum architecture

The manipulations listed above are related to surgical operations with varying difficulty, so they are implemented under local or general anaesthesia.

What the augmentation and plasty is about?

In case of gingivoplasty, soft tissue is augmented with a gingival graft.

In case of bone augmentation, a bone part on sub- or supra-maxilla is replanted with osteoplastic material.

Statistically, 35% of patients cannot have their teeth restoration through implantation without a preliminary bone grafting procedure.

1. Plastic periodontology

  • Receded gums plasty

Some periodontal diseases may cause receded gums: soft tissue mass decreases in density, resulting in the exposure of the tooth neck and a part of its roots. It provokes an acute sense of discomfort, completed by aesthetic defects. A neglected case requires gingivoplasty in order to cover tender tooth zones.

Aberrant fraenum attachment or small buccal vestibule can also contribute to gum deficiency: free gingival margin deviates, thus creating a gum pocket or evolving in receded gums. Plastic periodontology is also helpful in these cases.

Gingival recession coverage (gingivoplasty) aborts periodontal disease, eliminates dentin hypersensitivity during meals and fluids intake, prevents root caries and restores cosmetic comfort.

2. Aesthetic reconstruction

  • Gum plasty to eliminate cosmetic handicap

Gingivoplasty is the process by which either the gums are reshaped to correct deformities, or excessive soft tissues are extracted to create physiological gingival contours, in order to increase tooth volume and to make a more attractive smile. This surgery technique is widely used for a “gummy smile”, when prominent upper gums catch the eye, letting the teeth look smaller.

  • Gum plasty for implantation

Gingivoplasty for implants is carried out for one-stage operation or in case if the implantation is supposed to create a long missing tooth, meaning the presence of soft tissues craters. Surgical repair of the gum density protects the patients against periimplantitis — destructive inflammatory process affecting the soft and hard tissues surrounding dental implants.

3. Bone grafting in dentistry

  • Bone grafting for dental implantation

It is important to have enough width, height and depth of jaw bones where the implant is supposed to be placed. A deficient bone volume can result from:

  • Prolonged wearing of removable denture
  • Periodontitis and gingival inflammation
  • Cystis, inflammation of apex and maxillar sinusitis
  • Injury of the jaw
  • Bony rarefication
  • Innate anatomical peculiarity

It will enable your budget planning and the doctor’s scheduling the treatment stages to achieve the results you need.

  • Sinus-lifting

Sinus lift is performed when the floor of the sinus is too close to an area where dental implants are to be placed. This procedure is performed to ensure a secure place for the implants while protecting the sinus. In order to have an implant holding hard, superior jaw bone volume has to be at least 9 mm. If the volume or height is not enough, a surgeon will indicate a procedure called sinus-lifting. Osteoplasty is implemented in the posterior upper jaw with the sinus floor elevation procedure. Duration: 30 – 40 min.

Open and closed surgeries are possible:

  • Closed sinus lift is performed when a bone is wide enough to have a primary implant stability, but some volume needs to be added. The bone augumentation technique is done through the same apertura where a dental implant is placed, the same day. The method is the most patient-friendly and is normally followed by an easy recovery.
  • Open sinus lift is performed when a bone is not wide enough to have a primary implant stability. The surgeon will then need a wider access to add a grafting material. If the doctor adds little grafting material, the implantation will follow immediately. If some substantial volume is added, you will need to wait for 6 to 8 months, until your body integrates grafting material. Only after that procedure the implantation can be done.

After the surgery

The doctor prescribes pain-relieving and anti-inflammatory drugs and antibiotics, and provides you with a post-operative care kit.

The comfort of the rehabilitation period depends on the patient’s sense of responsibility. Healing lasts from several days to several weeks. In some cases of the bone grafting it can go up to several months. During the first 5 days can be present swollen mucosa, facial swelling and temperature rise.

General advice:

Have a rest, keep a low physical activity, have a soft room tempered food and avoid spicy food. Avoid tooth brushing in the place of surgery, practice a careful and meticulous oral care, including antiseptic mouth wash and timely medication prescribed by the doctor.
